Living Well with Chronic Disease Self-Management *ONLINE*

The Living Well Chronic Disease Self-Management Program for Pain is a self-management class for anyone with a chronic condition (arthritis, diabetes, heart disease, hypertension), or any other chronic condition that requires ongoing medication. Classes meet once a week for 2.5 hours and cover a variety of topics that support developing a healthier lifestyle and how to be a partner with your health care team. Registration is required.

All programs are led by a team of trained leaders and will take place online at this time. The program teaches the skills needed in the day-to-day management of treatment and to maintain and/or increase life's activities. The sessions are highly interactive and focus on building skills, shared experiences, and support. Participants create their own custom action plans and help each other solve problems they encounter in creating and carrying out their self-management program.
You must attend on of the first two sessions to be included in the six-week class.
People with different chronic conditions and their support people (family or friends).
This class is being offered entirely online during this time of social distancing. If you need assistance getting setup online, we will assist to ensure you can fully participate in the class.
Participants are required to have speakers and web camera and to have them on during the workshop.
We will be using the textbook "Living a Healthy Life with Chronic Conditions" for this course. You will receive a complimentary textbook upon completion of the course.
